Can you post a picture of the decal?

I suspect that this was made by Mike Redpath (who manufactures Falcon Propellers using the old Fahlin type certificates). Look for D566 on this page and I think it will probably match yours...

Looks like it fits a Lycoming 0-145 engine in any case.
