G.B. Lewis L11CK45

I just picked up a G.B. Lewis prop with the following identifiers:

DES L11CK45
SER 13876
TC 797

It is in overall decent shape. Full metal with a small patch of wood missing on one arm and perhaps an inch and a half of the other arm's metal and wood missing from tip. Decals legible and good coloring.

Can anyone tell me more about this? What type of plane it came off? Dates, value?
